Why it is separate from the will

Because you will change your mind. A memorandum referenced by your will can be updated without redrafting and re-executing the will itself, which keeps a small change from becoming a legal appointment.

What to list

Anything with a story attached. Be specific enough that a stranger reading it could identify the item and the person — full names, not 'my daughter'.
